Coz we can’t afford the article processing fees? Especially in areas which aren’t well grant funded. I’m not sure about a model where we are paying to publish our work also… In other industries authors get paid for their contribution but science and medicine has got it the other way round.

Patient factors remain the strongest predictors of anastomotic leak. Modest modifications to stapler technology seem unlikely to generate significant patient benefit and more radical and transformational changes may need to be explored.
