Fortuna died, yes. But what if Pandora and Petelguese took her body, resurrected her and gave her the Witch Factor of Wrath? She came back without her memories, so Petelguese gave her a new name (Sirius mentions she got hers from him), and, due to being incompatible with the WF, she's lost her mind.
