If they make social security payments they don't need the convenio because they can just register with the national health system (at least they can in the Canary Islands ... as I say, each area of Spain has devolved powers in health).

As you know, health is a devolved power in Spain and I can only say that in the Canaries, there's no copago, and there's no treatment for non-residents without prior payment on the spot in public hospitals. It could vary perhaps fairly considerably around the country.
