(I am trying to find the code I used to generate the lookup tables way back)
But yeah with the tables you simple sum them together (they can overlap) and end up with the full approximate waveform

The gist is that you have a set of N lookup tables, where each one corresponds to an "i/N" sub-sample offset of wave position (diagram from that site). You then generate your square wave rise/fall indices at sampleRate*N precision and use the subsample offset to look up which table to use
