The admin didn't have to do any of this. This could've been a funding announcement for a well powered multi center trial of leucovorin that considered key covariates. They could've pushed Congress behind the scenes to expand access to dietitian coverage for individuals with autism.

Sad for parents desperate for help & being heard online now paying for genetic tests, designer vitamins, & bending over backwards to feed restrictive diets they've been told are toxic. Sad for the kids w autism being fed restrictive diets, some of which will inevitably be imbalanced & cause harm.

The leucovorin story is already blending with existing misinformation online about folic acid and MTHFR genotypes to create a new generation of medicalized claims without much evidence.

FYI Jenny McCarthy is now out here promoting the Tylenol - Autism link citing its impact on glutathione and promoting IV infusions of glutathione.

and for folks who don't follow this, it is nearly all naturopaths, chiropractors and 'integrative' medicine folks who are pushing the glutathione rationale b/c they get to sell N-acetylcysteine & Vitamin C supplements (of which there are many poorly contextualized papers out there also hyping).

So many papers are clearly written to sex up impact and claim translatability, at the expense of accuracy - that's a gift to pseudoscience. We need to be training the next generation of scientists to be honest and media literate.

Every basic scientist & PhD program director should be watching how cell culture and rodent models of Tylenol exposure are being used by motivated parties (e.g. it depletes your glutathione! Science proves it PMID...) to sell a narrative and sow confusion right now.

It's very bizarre that the admin pursued a leucovorin indication for children with cerebral folate deficiency and autism symptoms, not for general autism. CFD is rare and the diagnosis is confirmed by measuring folate levels in cerebrospinal fluid, which most general cases of autism won't have.

As the admin continues to purport to have found environmental causes of autism (& pretend to care about nutrition), I find myself thinking a lot the failed National Children's Study. There's a push to have answers w/o reckoning w the reality of the studies we haven't done

Push back on leucovorin and folinic acid as a cure all. it is an early stage investigational therapy for a subset of children with autism. It is wild to me that the admin saying public health damaged itself during COVID is now set to overstate the efficacy of a treatment for autism.

- Dosing will be a mess because folate is listed as micrograms and its in both the amount of the form used and dietary folate equivalents

-Folinic acid comes as isomers (D- and L-) with only the L- being biologically active. The pharma version are 1:1 D- and L-. Most supps don't even state the mix

With the inevitable coming shortages of leuocovorin, there's going to be a huge push for influencers (& folks in our government) to profit off of folinic acid supplements. A few things:

-doses used in trials are pharmacological and the OTC supplemental versions will last a few days to maybe a week.

There's going to be a push to present this as a cure all when there are deep uncertainties and the preliminary evidence already only points to sunsets of pts that are likely to benefit
