We have an ice cream stand that has two windows. One for ice cream, one for food. You must wait in line, order, and pay twice. On quiet days, the same lady operates both windows.

Anecdotally, I grew up riding, showing hunter jumpers, and no one was ever injured seriously. I did end up in the ER after getting hit by a car while riding my bike, though. (I was fine, she slow rolled through a stop sign right into me)
