PS: The cited LD50 value of 1–3 mg/kg is dubious. The paper [2] presents this range in its literature review, but the referenced source states: "One source suggests that 1–1.5 mg can be fatal in humans [14] and another suggests an oral LD50 of 3.16 mg/kg [20]."

Coconut-fermented products feel like an unnecessary risk. If there’s a BA poisoning case (in Europe), would the hospital staff even acknowledge BA poisoning in their differential diagnosis? Would a rapidly comatose patient ever be linked to a cup of joe?

Relevant to my personal interests, there are coffee beans co-fermented with coconut. These coffee beans are sold as specialty coffees globally, also by European coffee roasters. But just to be clear: I'm not aware of any BA poisoning cases linked to coffee products.

As the outlook for BA poisoning treatment remains grim, strict hygienic practices and monitoring efforts should be adhered to. Yet, e.g., the production of tempe bongkrek has been banned in Indonesia since 1988 [6].

Generally, BA production is favourable in environments with neutral pH, mild temperatures, low NaCl concentration, and rich in fatty acids [2]. However, contamination with B. gladioli pv. cocovenenans is still a necessary prerequisite.

Should you be interested in cooking with products that have a non-zero chance of containing BA, please be advised that BA is thermally stable [5]. For example, a study highlighted via [4] reports that an hour-long exposure to 120 °C temperature was incapable to inactivate the toxin.

No antidote for BA poisoning exists. BA interferes with cellular energy production, which results in cell damage and death [3]. A non-zero amount of BA results in grave danger to all critical organs [4].

BA poisoning has a high mortality rate, and the fatal dose for humans is around 1–3 mg/kg [2]. For an 80-kg human, that fatal dose would roughly be the same mass as a pinch of salt. Yet, there’s no safe dose of BA.
