And then at level 80, you unlock Living Shadow, where you literally manifest Fray/Esteem to fight alongside you.
Tl;dr nobody can see Fray until you manifest them from lv70 onwards. Prior to that, any reference to Fray is actually YOU, the player.

Myste is your Survivor's Guilt. Myste IS physically present, alongside the ONE instant of Fray/Esteem being physically present when you manifest them in the lv70 DRK capstone quest. We know this because Sid interacts with both- he calls Myste a 'mewling whelp' and throws his Deathbringer to Fray.

We get the Midlander M placeholder Fray for convenience, and I suppose engine limitations. But there are multiple instances where Fray is literally just a copy of your character wearing different armour in cutscenes.

When other people refer to 'her' they're referring to YOU. Nobody else can see Fray other than OG Fray's corpse in the Brume. Only you can see them otherwise. All the actions 'Fray' takes? It's you.
