Crunchy has some shaaaady workflows for non-English subs, I can vouch for that…
As for Disney, tbh I’ve never done any anime with them so I can’t say. (I’m not a fan of Disney+’s quality on any of their localised content tho, they don’t invest on it and it shows)

Depending on the platform, of course. Most Netflix projects I’ve worked on had two completely different adaptations, same as some anime where the production took care of the JA to EN work before sending the content off to get localised in any other languages for the target platform release.

The result is still quite different, unless nobody in production cares about quality at all and they just auto generate subs by feeding the dub script into an app or sth.

Usually it’s two different translations. if the content owner is a cheapscate, theyll try to save corners by having the dub use the subs as a base, or the subtitler to adjust the dub script to fit into subs.
