I bet there will be studies looking at post-65 initiators of HRT now that the opinion tide has turned on HRT in clinical medicine. I predict that in fit and healthy women, the benefits of low-dose transdermal estradiol (with or without progesterone) will be found to outweigh the risks.

Combination hrt is necessary for women with an intact uterus bc estrogen alone causes uterine hyperplasia and increased endometrial cancer risk. Progestogens protect the endometrium but also reduce (but do not eliminate!) the benefits conferred by estrogen.

Evidence from this elegant and ground-breaking paper supports unopposed transdermal estrogen as conferring far greater health outcomes than estrogen and progesterone in combination. The hitch is one would need to have had a hysterectomy…
