Sorta depressing paper. Mortality is high in all patients regardless of ICD device platform. S-ICD was likely selected due to vascular access problems and concern for potential future bacteremia, but no sig difference in hard outcomes.

Tbh, I would have called inferior STEMI based on presentation EKG if combined with CP...
I probably would have guessed wrong on RCA vs LCx... but important to note that traditional EKG vectorography falls apart s/p prior MI or CABG.
In my mind, LCx makes sense give isoelectric lead I.

@cardioobdoc.bsky.social I have to say that I respectfully disagree with the proposed "fast track" management protocol, which does not address new LBBB s/p TAVR.
LBBB is common. Per paper, 10% required PPM in first year, 50% of those within 30d of TAVR. #CardioSky #EPeeps
