We identified 740 high-engagement, regulator-relevant social media posts that had amassed more than 57.5 million views as of January 2025. Patients/consumers and lifestyle/celebrity influencers were the most prominent sources across all platforms Overall, 175 posts (23.6%) used promotional language with prevalence ranging from 14.4% on TikTok to 33.0% on Instagram, and creators often describing drugs as “life-changing” or “revolutionary.” Further, 116 posts (15.7%) included specific calls to action (eg, “ask your doctor,” “visit our website to get started”)….Despite these marketing elements, explicit disclosures of sponsorship using sponsored tags (eg, #ad, #sponsored) were rare, appearing in only 16 posts (2.2%)…When combining all factors, undisclosed influencer promotion was present in at least 593 (80.1%) of these high-engagement social media posts.
