One of the maps contain what i assume is the bathroom from Saw 1, and the axe has been reskinned to a iron pipe giving a cool industrial survival horror vibe (both visible in image 4).

This map pack does a amazing job at displaying this phenomenon. Fittingly a monster squid has been added to the enemy roster. That setting in image 3 feels unique and not something I've ever seen in Quake before. continued below...

It runs on the "Quoth" mod (loads automatically) which adds a ton of new enemies including flamethrower guards and an extremely tall boss monster that will lift you up and throw you away, it looks very unsettling and the sound it makes! Sounds like a mix of a whale and a warning siren! more below...

This set is very cadaveric. The levels feel extremely damp and muggy! The usage of some of these textures is very nauseating. Brutalism and shady facilities are themes occasionally present as well. continued below...
