Typically I preO2, while I give a weight based does of midaz and opiate, I use propofol Marsh model, set the target at 6, instruct my assistant to apply cricoid and give roc when the target is reached, I wait 60-90s, I tube.
(Of course it's more a fixed sequence induction than a true RSI)

A difficult case.
There's a blurred line between "doctors could have picked up" a rare diagnosis and "doctors should have picked up" a rare diagnosis.
But it's not unreasonable to assume a pt presenting with a grossly swollen eye to a university hospital should have been seen by the eye team ASAP.
