An important new development at NCISH. We are resuming detailed data collection into patient homicide. This programme will expand on the national homicide inquiry that ended in 2018. We plan to explore new questions, additional methodologies & work closely with affected families.

We found numerous factors that may have contributed to suicide and that should be the target of future prevention. Two stood out: mental ill-health and academic problems. Several other factors (e.g., neurodiversity, financial problems) point to the need for targeted support.

Our aims were to collate key findings identified within these serious incident reports, assess the quality of serious incident reports against sector-wide & @ncish.bsky.social guidance for investigating serious incidents & identify examples of good practice and areas needing improvement.

Our most recent report is our national review of higher education student suicide deaths. We examined serious incident reports of suspected suicide deaths &incidents of non-fatal self-harm submitted by higher education providers (2003 & 2004)

The ward teams present at the event reflected on their own practice and how to include lived experience perspectives in their new approach to risk. The afternoon ended with a panel discussion facilitated by Leah, Eloise and Charlotte.

Nina & Julie spoke about Derbyshire Healthcare NHS Foundation Trust's co-produced Suicide Prevention, Safety Planning & Risk Assessment Training. Darren from Sheffield Health & Social Care NHS Foundation Trust provided an update on the Trust’s progress with moving away from risk stratification.
