As I argued at a panel in 2023, the "trans question" is rooted fundamentally in anxieties about threats that democratic egalitarian society that permits trans people (and lgbq+) will not achieve the reproductive goals of white western conservatives.

It would be very nice if being trans wasn't a constant exercise in 'i'm telling you what i want and need' and a medical professional going 'i think you are confused as this is really about what i want and you get what i want actually'
