Have a try with this image as bokeh texture in front of the cam.
You just have to scale the image up / down to change the effect.

Quick try:
darken your room, display the image on your screen and put your hands on the table. NOW watch your hands shadows on the table.

This works like a charme and unbeatable with boxcutter and such...
