I have little doubt that this is an infusion site. Using the back of the hand allows the patient to have the use of the hand during the procedure (he can still click the remote).

A Christmas Story. The dad reminds me of my dad, esp trying to fix the coal furnace. And the fra-jee-lay lamp. I still laugh at the little brother in the snow suit. We watch it every year.
