The jury concluded that multiple failures possibly contributed to Ben’s death & that aspects of Ben’s treatment in the community, prior to him being sectioned, had been inadequate. A decision as to whether the Coroner will make a PFD report in this case is awaited. Continue reading: bit.ly/43bvYj9

Olivia Beach succeeded in a Section 204 appeal, which raised complex issues concerning jurisdiction. The appeal was allowed on the basis that the Local Authority who accepted that the conditions for referral were met, were not involved in the review decision

The jury concluded that there had been missed opportunities to intervene and support Alex appropriately. The jury also concluded that there was insufficient policy and procedure in place in respect of Method of Entry kits, leading to an ‘unacceptable’ delay.
