But this change would: 1. Add to the cost of healthcare bigly and totally unnecessarily. 2. Add to the fragmentation healthcare, ie, more hassle for parents, and 3. Greatly improve the odds of children not getting all three.

Massive influence via political pressure and contributions. The result is even minor adjustments to reduce costs are thwarted. Either the US is doomed to a national health care bankruptcy, or all of our politicians need to put US first & their political careers 2nd.
