Comparison of size and key pharmacological properties of nanobodies with conventional antibodies (IgGs) and small molecules. For brain targeting, they combine the high selectivity and solubility of antibodies with the receptor modulation capabilities of small molecules. Despite a limited access to the brain, nanobodies are expected to have a better brain penetrance that IgGs. A recent study has established the proof-of-concept that a nanobody targeting a neuroreceptor restore cognition in preclinical models of schizophrenia (A) Mice were treated with a biparatopic nanobody that behaves as a positive allosteric modulator specific for the metabotropic glutamate receptor mGlu2 homodimer, which is considered a target to reduce glutamatergic tone in brain regions related to schizophrenia. It enters the brain after intraperitoneal injection. (B) This nanobody restores cognitive deficits in two distinct mouse models mimicking NMDA receptor hypofunction ; a key endophenotype of schizophrenia. (C) The nanobody binds to presynaptic mGlu2 receptors, reducing glutamate release into the synaptic cleft, underlying the observed rescue of cognitive de fi cits in treated mice. (D) Preliminary evidence shows that nanobodies penetrate the brain by extravasation at fenestrated blood vessels, located in the circumventricular organs.
