If you've ever had a name change for any reason, PLEASE make sure you have documentation of all name changes- even if you changed your name back. Get a pocket folder and fireproof safe or pouch for everything. Keep it all organized and accessible.

If the utilities are entirely in your partner's name, consider switching at least one into your own name, or see if both names can be on the account. At age 16, minors need a state issued photo ID. They should also receive a student ID at school. Non-citizen documentation, too.

You need your birth certificate, state issued photo ID (like a driver's license) or military ID, your marriage license, divorce decree, and social security card. If you have kids, get their documents in order, as well. Your rental agreement, mortgage paperwork, recent utility bills in your name....

Please do get all your documents together, and if you can figure out a way to afford it, please do get a passport. Just in case. Maybe you won't need it- but better to have it and not need it than need it and not have it.

I applied at my local post office this morning, and they did not return my birth certificate. The clerk explained that it is required to mail the original. The website did state this as well, but I thought I HAD to be misunderstanding/reading it wrong.
