💔 In HF patients with loop diuretic resistance, therapy depends on mechanism (DR-Loop vs DR-Distal).

📊 JASN RCT (n=103):
• ↑ Loop dose → +49 mmol Na excretion
• + Chlorothiazide → +107 mmol Na excretion

👉 Thiazide add-on more effective overall.

In a secondary analysis of 7,562 Chinese adults with eGFR 60–90, intensive BP control reduced composite CVD outcomes by 43% vs usual care, with no significant difference in kidney function decline.

Lower secretion = ATN risk ↑ (OR 0.34 per SD; AUC 0.73).

📊 Of 1,226 studies screened, 24 RCTs (n=3,906) on HDF vs HD were analyzed.
🔹Most 4h, 3x/wk, post-dilution (15–24L effluent). Symptoms studied: cramps, fatigue, GI, itch, restless legs, sleep.
