I just wrote a podcast about Flixborough. The repairs they made to a system carrying highly flammable chemicals were cavalier at best. 28 workers died, but it could easily have been hundreds. No one was prosecuted. In a H&S vacuum working people pay the price. podcasts.apple.com/gb/podcast/c...
