It depends on what you mean by low income. I just looked into this today, You have to make less than 90,000 a year and can’t have dental insurance already so that will apply for most people without dental care. It’s still not fully universal, but it is a way of insuring that the most people have it

To be fair (which you shouldn’t be for both the tomb of horrors or Gary gygax), the point of tomb of horrors was basically to bully and fuck with power gamers at conventions. So he wants to make the players feel dumb for not knowing these colours.
