I wear mediums because I am a tiny build, but I have issues with certain areas being uncomfortably tight due to anatomy if I wear smalls. I SWIM in mediums in most brands, it's horrendous. I've had the best fit with medium mega maxes, and to get that "diaper shaped" profile I just layer if needed
