I think that's in breach of contract unless they explain (which is kinda irrelevant to you) that they will be doing the form for you and you're free to phone, walk in and book appointments as you used to. If you want to email me abt it I'm, on [email protected]

Laurie, did your GP say that the form is the only route to access GP. If so they are breaking their contract.

Yea a mix of stories. some v positive, and some where surgeries are blatantly breaching contract
