Expulsion from office is the only mechanism for removing a member. Each chamber would have to make their own rules and in today’s world, one’s health privacy is unlikely to be breached in this type of situation. They’d likely be breaking the law just by holding a vote on one’s cognitive abilities.

By forcing retirement, that automatically puts younger representatives to work. That’s why a three term limit is reasonable. Two terms is definitely not enough to be effective. One elected to the house could serve 6, then go to the Senate serve 18, maybe even president and then get another 8.
