You also get to learn from DGH consultants who have managed as whole stay specialists for 20+ years with limited access to cath labs, nerve conduction studies and other such dark magic and manage on clinical acumen.

Further into the hospital I can't comment but I'd be surprised there aren't advantages to being say the "generalist" ophthalmologist in a DGH over being part of the retinal team.

From acute it's easier to get changes in place without the interference of a gastro professor who does receiving three times a year, but has more clout than the folk constantly at the coalface.

I think the closer you are to the front door the more you can get out of a DGH. From an ED perspective not having every specialty on site to rescue you enforces a skillset.

Nearly every ED will have, in living memory, a day where demand was high due to ice, but flow was fine due to ice keeping the sick from leaving home. ED will have seen ~50% more patients than usual, pulled more wrists than they do in a week and send most home. Demand can be met if flow is OK
