“Bulging Fissure Sign”

A radiological finding that occurs when the affected portion of the lung expands, displacing the adjacent fissure.

It's often seen in the right upper lobe (RUL) of the lung and is classically associated with Klebsiella pneumonia.

The bulging fissure sign is less commonly seen today due to the prompt administration of antibiotics to patients with suspected pneumonia.

To confirm a diagnosis, a bronchoscopy is an important tool because of the endobronchial nature of the pathophysiology.

However, it can also be caused by other types of pneumonia, or by other space-occupying processes in the lung:

Streptococcus pneumoniae
Pseudomonas aeruginosa
Staphylococcus aureus
Legionella pneumophila
