Sound the alarm: prevention is far cheaper — and kinder — than crisis care.
📊 Evidence is from a TB Specific Impact Survey, conducted during the Stop TB Work Orders, with 180 responses from 39 countries.

We’re effectively inviting TB to spread unchecked. Every prevented case averts multiple downstream infections — the multiplier we can’t afford to lose.
These aren’t routine budget adjustments; they’re decisions that set the stage for future epidemics.

When prevention is cut, more people are exposed, more infections follow, and the risk of drug resistance grows.
Who loses access most:
● Health workers with frequent exposure

● Children in households affected by TB

● People living with HIV

● People in close contact with someone with TB

This isn’t “just a service change” — it’s a breakdown in life-saving continuity of care.

Picture this: You’re three months into a six-month regimen, starting to feel better. Then the message arrives: “Program paused.”

Without consistent medicines, monitoring, and support, many don’t complete treatment, resistance can develop, and communities face preventable risk.

Why this is so dangerous:
● Incomplete treatment → higher risk of relapse
● Ongoing transmission in households and communities
● Emergence and spread of drug-resistant TB (including MDR/XDR)
● Avoidable harm and preventable deaths

Kendall's a true ally in the fight to #EndTB too...big fans of hers and her work here.

...and I stress the importance of putting affected communities at the center of the fight against TB and what I see as the missing bits that we need to put together in order to end TB, if not by 2030, then as soon as possible. I hope that you will join and share with anyone who is interested."

My talk is Oct 14 and is pre-recorded due to time zone differences and will cover our progress towards TB elimination by 2030 as well as some of the fallout from the USAID cuts. I tried to hit the right balance of optimism and an honest assessment of where we are today ...

We’re not just delaying care — we’re building the next drug-resistant TB crisis.

Then: “We can’t initiate treatment right now.”

This is happening to thousands every day.

When treatment can’t start:
● Illness worsens

● Transmission continues in households and communities

● Drug resistance emerges and spreads (including XDR-TB)

● Preventable deaths occur while waiting

We’re creating diagnostic blind spots where TB spreads unseen.
This isn’t “just logistics”—it’s life-saving infrastructure for TB prevention and care.
