If you don’t prioritize… your interventions miss the target.

If your interventions miss the target…your revenue + outcomes don’t improve.

Big systems can afford mistakes. Small + rural clinics can’t.
-overlooked diagnosis
- silent crippling pattern
- uncontrolled metrics
can knock you out with 1-2 max 9% penalties.
and the penalties are coming from everywhere —not just Medicare.

Once you simplify & combine all the MIPS & HEDIS measures and see where the overlaps and gaps are, the next question is: Which of these gaps matter most? This is where outcome risk measurement comes in.

Two quality measures can look identical on paper—same gap, same performance.

But when you score how bad it is, how often, awareness, effected patient volume, cost impact, and systemic reach…

only then can you see the TRUE impact quietly bleeding your clinic.

You can “improve” a metric & still get financially crushed, under value based care.
If the intervention isn’t targeting the patient population or diagnosis driving your cost + utilization…all you’re essentially doing in reality is rearranging the furniture while the house is on fire #valuebasedcare

Whether we agree with the quality programs or not. Having a large uncontrolled patient population that is consistently in the ER or hospital across several different diagnosis is still not ok.

You are absolutely getting penalized for what you don’t track and measure.

If you’re pushing mammograms while your biggest population is diabetics with high ER use… the penalty isn’t coming from where you’re looking.

If you’re just now looking to see if you’re MIPS eligible in December. The month of submission deadlines I can almost guarantee you’re probably going to be penalized.

Under fee-for-service, you got paid for what you did.

Under value-based care, you’re being paid for:
your outcomes
your risks
your documentation
your patient behavior
your care coordination
your readmissions
your ER visits
