Vascular dementia progression is uneven and patient-dependent. One fairly common feature is decline in fits and starts, followed by periods of relative stability. Additional cerebrovascular events, like TIAs, accelerate the decline.

Similar to what I teach at the med school & residency level about documentation: writing clear and distinct notes helps clarify & hone clinical reasoning.

But the pressures of the work and electronic medical record format have reduced it to a rote task rather than a useful exercise.

I’m all but certain he has cerebrovascular disease. Whether he’s having recurring episodes like TIAs or recrudescent symptoms due to stress is unclear. But it’s clearly taking a toll.

True story: when I was a senior night resident covering the CCU, sitting at work station at like 3 AM, I heard a loud moan like an elderly patient from what I new was an empty room. I looked at the nearest nurse and she was like “oh yeah that’s our ghost. First time?”

Kicking millions of people off of Medicaid will hurt the entire healthcare system. It won’t just hurt people they don’t like and it’s incredibly stupid to pretend that that it will.

He knows he’s a fascist. Behind closed doors, he’s proud of it. But he also knows it’s a branding problem. Still, he’s undisciplined when it comes to media, so things like this leak through.
