It's very common with /f/ in some regions, yes! (That's /pfaˠː/ in my dialect, though. So I'm out. ;) )

Not to my knowledge (which doesn't mean there aren't any, but if there are, it's a very local phenomenon). There are many dialects that still have the /p/ in that word.
