In my current game, the character either suffers a temporary injury or 1d6 system strain (player choice). If they roll the same injury twice, the injury becomes permanent. 1 system strain is required for most free and easy sources of healing, and your max = your Con score.
The second is "What happens when you have 0HP (or equivalent)?" You can always add "The player *chooses* if they want their PC to die" as an optional rule.
