Not always external. As I said, the PDS operator has moderation power, they can create, edit or delete data, allow or revoke user access to the PDS, and suspend accounts.
Moderation can occur at the PDS-level, and it's even good to do for some kinds of problems.

Sure, I could host my own PDS and do whatever I want, but it doesn't stop me from facing consequences

Pra quem ta no Fedi, é só ter um "Fediverse Attachment" no seu perfil com o título exato de "Pronouns" que o Wafrn puxa dali
Pra contas do Bluesky, é usar alguns dos clients alternativos que permitem edição da propriedade "pronouns" no perfil (como o client do Blacksky)

Could be a spam / bot PDS that was blocked by Bluesky at their Relay, but I'm didn't look up more deeply to be sure yet
There are a bunch of spammy data you can fetch while crawling PDSes, even non-existent PDSes that are faking data
