The "doesn't increase pH unless increase in ventilation is a myth": pubmed.ncbi.nlm.nih.gov/10923730/ Temporary increase in CO2 production = temporary increase in paCO2, which goes back to baseline without change in minute ventilation, maintaining an increase in pH that's there from the start.

Is it accurate though? Sodium is only a minor contributor to the mass of the drug, e.g. 1 gram of ampicillin contains 62 mg sodium = 2.7 mmol. So there's only a sodium problem if you dissolve it in high volume of NaCl 0,9%, which isn't necessary (at least not for amoxi/flucloxacillin which I use).

The formula can be simplified to 5 mL/kg NaCl 3% which will result in an expected increase of about 3 mM in men and about 4 mM in women. Or 0,5 mL/kg NaCl 23.4% again for an expected 3 mM increase in men and 4 mM in women. Holds quite well for starting [Na] 110-150 mM and easy to calculate.

In my experience i.v. does lower the blood pressure quite well without causing a significant drop in heart rate. So I'm surprised by this. What's your experience? Maybe the composition varies by manufacturer?
