<me, reading about New Delhi CRE>

"Man, we don't even have MRSA, here."

Mind-blowing, still.

The bizarre weather where the temperature goes up 5C at sunset.
A graph of hourly temperatures than goes up when then sun sets.

Okay, they’ll all be aspiring grifters née “creators”, amplifying the message of those with money and power for a chance at being tossed scraps from their table.

Regressing to the mean! Crisis over. The planet is healing.

You no longer have to hide in shame if you've been giving kids ondansetron all along.

It works at home the way it works in the ED – a little bit, and a little bit is sometimes enough to make a difference.

You Can Keep Giving Kids Ondansetron
You're not doing any harm, and it probably helps a little bit.

But if they’re a product of a broken home they’ll grow up to be rapscallions.

I’ve had a theory that “authenticity” will be the most valued construct for the current generation growing up.

As artificially-generated content explodes, verifiable human creations will become increasingly treasured.

Well gang, it's happened: OpenAI has built a platform filled exclusively with fake, AI content.

I've been playing with it for a few days, and I think it's safe to say we're basically kissing reality goodbye.

Here's the story: www.npr.org/2025/10/03/n...

And some thoughts 🧵

I think my bias shows through my general bemusement at the relative Australasian obsession with a-lines …
www.evidencetriage.com
Hm ... I won't necessarily quibble with an a-line in a stabilised, critically-unwell patient awaiting transport from a rural centre, but I have a hard time justifying the value of twiddling with a non-therapeutic intra-arrest procedure with only the hope it will make other interventions better.

The answer to the “why won’t anyone buy our startup’s great model” question — all the best data science will gather dust if you can’t solve the rest of the equation for the necessary stakeholders.

what’s the excuse now and you can’t blame the wind today because it’s uncannily still

If you’re excited about non-specific markers of infection claiming to be the next great (and $$$) thing in “sepsis diagnosis”, then you’ll be pumped for TriVerity.

TriVerity: A More Expensive Procalcitonin
... which, itself, is a more expensive CRP.

Everyone who presents to a U.S. emergency department is federally mandated to receive care as per EMTALA c 1986, regardless of status. This is either paid for by emergency Medicaid or the hospital eats it (and cost-shifts it, ultimately).

Nothing in the current shutdown conversation relates to it.

Vance: "If you're an American citizen & you've been to the hospital in the last few years, you've probably noticed wait times are especially large & very often somebody who's there in the ER is an illegal alien. Why do those people get healthcare benefits at hospitals paid for by American citizens?"

I get chloride reported with all my tests in the hospital; it's soooooo nice. Not very useful, just the warm comfort of an old friend.

Companies pushing fringe treatments (see: testosterone supplementation, various 'longevity medicine' treatments) have no incentive to test their therapies for efficacy, because then the general medical establishment would start offering them, cutting into their market share ... 🤔

"Doctors don't report what patients tell them" is actually "doctors don't record ICD-10 codes for all the symptoms reported".

Not as exciting as I hoped when I started reading it, with fewer direct clinical implications.

Doctors Don't Report What Patients Tell Them?
... or is it really just lost in translation.
