If a 16 year old cisgender boy with gynecomastia is distressed by the size of his breast tissue (and all cisgender men have breast tissue!), he can have that corrected with no fearmongering! If a transgender 16 year old boy is, in many locations he has absolutely no recourse.

These laws all specifically exempt "bilateral mastectomy for treating gynecomastia in cisgender boys" and "genital surgery on infants and young children to 'correct' intersex conditions" (scare quotes because it's a perfectly normal human variation that doesn't need 'correction' in 99% of cases).
