Your single best "we could remove this with very little damage to the system as it is almost completely vestigal and it would see meaningful cost decreases" are PBMs.

But very few people even know PBMs exist, and they like to keep it that way.

Signed: @scottwiener.bsky.social’s SB 41 to regulate PBMs (prescription middlemen). Newsom had previously vetoed a broader version, SB 966, in 2024, but included provisions for licensing PBMs through the state in the 2025 budget. This bill would add additional regulatory guardrails.

That's just a happy side effect, they sell user data and cut deals with the Pharmacy Benefit Managers (PBMs) who are notorious for being a major part of the ridiculously inflated medicine prices. This also hurts small independent pharmacies.

It's just taking advantage of the systems already there.

former EMT here to say this is a fucking asinine take! immigrants did not build a for profit health care system with immense administrative bloat, nor did they invent insurance companies, PBMs, or other made up middlemen who screw people out of needed care that could keep them out of the ER

—Bans “spread pricing,” where PBMs charge health plans more than they pay pharmacies & pocket the difference

—Bans PBMs from pocketing rebates — PBMs now make more when they favor higher cost drugs (rebates are often a percentage of drug costs & PBMs get a percentage of these jacked up rebates)

Bans PBMs from forcing/manipulating people to use the PBMs’ own mail-order pharmacies instead of neighborhood pharmacies, which kills pharmacies & eliminates consumer choice. It’s a super anti-competitive practices that makes health care worse for patients & harms neighborhoods.

It hasn’t gotten a ton of attention, but I have a bill on the Governor’s desk to ban the most abusive & rent-seeking practices by pharmacy benefit managers (PBMs) that drive up Rx drug costs & kill off pharmacies.

Our state Board of Pharmacy put out a notice that unless the ACIP changes guidelines, all current protocols that are in place will stand and we can continue to give the vaccines as previously arranged. Now to make sure the PBMs pay!

Not true. We have a growing number of brand meds. The only reason we don’t have all brand meds is because the big PBMs threaten to reduce sales of the brand’s meds if they work with us.

They know we will do to brand pricing what we did to generics.
Unfortunately Cuban can only work his magic on generic drugs. Patent holders control the pricing of their own drugs.
