为眼科从业者建立一套标准化的干眼病 (Dry eye disease, DED)诊断方法, 对于消除患者疑虑,确保不同从业者诊断结果的一致性, 以及向政府部门提供干眼的真实患病率和相应医疗需求至关重要.国际泪膜与眼表学会 (Tear Film & Ocular Surface Society, TFOS) 第三届干眼国际专家共识 (Dry Eye Workshop, DEWS III) 回顾了自2017年报告发布以来的循证依据, 并修订了干眼定义:“干眼是一种多因素,有症状的疾病, 其特征是泪膜和/或眼表稳态失衡, 其中泪膜不稳定和高渗透压,眼表炎症和损伤以及神经感觉异常是其病因学因素.” 该定义的关键特征包括:干眼是一种多因素导致的疾病而非综合征, 并且始终伴有相关症状.本报告提供了干眼的鉴别诊断和眼科检查指南, 并列出了应告知患者的危险因素.本报告推荐采用OSDI-6作为筛查问卷, 其阳性临界值为≥4分.问卷筛查结果为阳性并伴有非侵入性泪膜破裂时间 (non-invasive breakup time, NIBUT) 8 mOsm/L) , 即可诊断为干眼.此外, 应对眼表进行染色检查, 有干眼症状并伴有角膜荧光素点状染色>5个;和/或结膜丽丝胺绿点状染色>9个;和/或睑缘丽丝胺绿染色长度≥2mm且宽度≥25%, 也可诊断为干眼.本报告依据泪膜相关因素 (脂质,水液和黏蛋白/糖萼) 和眼表及附属器相关因素 (解剖结构对位不良,瞬目/眼睑闭合异常,睑缘异常,神经功能障碍,眼表细胞损伤/破坏以及原发性炎症/氧化应激) 对干眼亚型进行了分类, 并为识别个体患者的这些病因的驱动因素提供了相应的临床检查方法和诊断界值, 从而为制定恰当的管理和治疗方案提供指导.

A good chunk of the reason I'm so open and up front about being trans and especially in my scicomm work is because I've had people like y'all showing me just how much it makes a difference to have people being out and loud and proud. I want to pay it forward and be like that for someone.

Had a very happy morning at the #Kirkcudbright Dark Space Planetarium this week. A fab interactive exhibit plus a gorgeous immersive experience in the planetarium. It's so unique to have such a state-of-the-art science centre in this 🏴󠁧󠁢󠁳󠁣󠁴󠁿 rural setting, and the team there are just awesome. #scicomm 🔭
