one of my favorite things about James Cameron is he included many of the technical specs necessary to not die in a submarine at the beginning of Titantic.

OceanGate ignored all of these btws

Of the many, many gobsmacking aspects of the NTSB report on the OceanGate Titan's implosion, it was this relatively straightforward one that made my entire head explode like the guy in Scanners.

They left it in a parking lot. Outside. Through an entire winter.

From what I could get through the theme is that they were trying to do something novel and interesting and risky and so that required extra precautions. What oceangate did was the opposite of that.

To be fair, the NTSB redacts all identifying information from its reports, even if literally everyone knows who they are. The pilot of the OceanGate Titan had his name redacted too, as were the crew of the El Faro.
