one of my favorite things about James Cameron is he included many of the technical specs necessary to not die in a submarine at the beginning of Titantic.

However, OceanGate did not evaluate cycle life for the v2 Titan composite pressure vessel. OceanGate performed stress analysis on a carbon fiber composite pressure vessel, with all results (both v1 and v2 designs) indicating that a carbon fiber composite pressure vessel would have significant safety margin at the depth of the Titanic wreckage. However, these analyses were based on theoretical models of composite structures that assumed perfect geometry and fiber orientation, rather than the actual carbon fiber composite pressure vessel built by OceanGate.

Of the many, many gobsmacking aspects of the NTSB report on the OceanGate Titan's implosion, it was this relatively straightforward one that made my entire head explode like the guy in Scanners.

They left it in a parking lot. Outside. Through an entire winter.
OceanGate made multiple changes to their handling of the Titan that could have caused additional damage between the end of the 2022 expedition (dive 83) and the casualty dive. Dive data from the casualty dive were lost during the implosion, so the extent of additional damage to the pressure vessel at that time (or during the 2023 expedition) is unknown. The Titan was stored outside after the 2022 expedition until the early spring of 2023, subjecting it to freezing temperatures. If water had penetrated any part of the pressure vessel and frozen, the expansion that occurs due to the phase change to ice could have caused additional damage.

Oh my god. The scale models kept imploding so he's like "don't worry I have a new manufacturing method that will stop these implosions" and they're like "should we test a scale model built with this new method" and he's like "no no just try it on the real sub I'm sure it'll work"

OceanGate determined that wrinkles in the cylinder had likely caused the sub-scale models' failure and identified the co-bonding process as a way to potentially mitigate wrinkle-related failures. However, OceanGate did not build or test any new sub-scale models using the co-bonding process; the only article produced using the co-bonding process was the v2 full-scale cylinder ultimately used in the casualty dive.

OceanGate reminds me of a friend of my mom's that my dad avoided on the grounds that everything always happened to her, and he didn't want to be anywhere nearby when it did. Just in case.

From what I could get through the theme is that they were trying to do something novel and interesting and risky and so that required extra precautions. What oceangate did was the opposite of that.

The optimized design in Boeing's feasibility study specified a CFRP hull thickness of 7 inches. However, OceanGate reduced the final TITAN’s design thickness to 5 inches, with a key difference being the exclusion of 45˚ plies from the final CFRP layup. Boeing’s preliminary report had indicated that allowable strain tables did not support layups without plies oriented at 45˚. Despite this, the final TITAN design utilized only circumferential and axial CFRP plies, yet no one involved in the project interviewed by the MBI could explain why the 45˚ plies were excluded. In addition, a Boeing Material and Process Engineer involved in the feasibility study provided testimony to the MBI confirming that Boeing was not consulted or involved in any discussion with OceanGate related to OceanGate’s decision to exclude the 45˚ plies from the final TITAN hull design.

That's not even one of the most messed up parts of the investigation. There's one part in the coast guard report where rush had gamer rage.

In testimony regarding the ANDREA DORIA expedition, the OceanGate Director of Operations, who served as the assistant pilot to Mr. Rush, described a critical moment during a dive when the CYCLOPS I became stuck under the bow of the ANDREA DORIA wreckage. The assistant pilot stated that Mr. Rush experienced a "meltdown" and refused to let him assist in resolving the situation. When a mission specialist suggested that Mr. Rush hand over the controller to the assistant pilot, the assistant pilot reported that the controller was thrown at him. Upon obtaining the controller, the assistant pilot was able to free the CYCLOPS I from the wreckage and safely navigate it back to support vessel WARREN JR.

To be fair, the NTSB redacts all identifying information from its reports, even if literally everyone knows who they are. The pilot of the OceanGate Titan had his name redacted too, as were the crew of the El Faro.
