It's called a "hexaflexagon", there are various instructions online so just search and find one that works for you (I'd definitely start with a paper mock-up to make sure your panels are oriented correctly!)

I've made these out of paper before. Never from quilting supplies. It looks like each triangular panel is stiffened, but other than that it should be pretty similar to the paper version, I think.
