As states around the country are mandated by HR1 to rush to implement Medicaid work requirements by HR 1 the risk for waste of taxpayer $$ to corporate vendors is huge.

New paper w/ Brian McGarry, Ashvin Gandhi, and Drew Wilcock in @jamainternalmed.com!

Hospitals are complaining across the US that patients are "stuck" waiting for rehab beds at nursing homes when they are medically stable and ready for discharge. What is going on??

jamanetwork.com/journals/jam...
Question Has hospital length of stay increased more for Medicare
Advantage beneficiaries than for traditional Medicare beneficiaries
since the COVID-19 pandemic?Findings In this cohort study involving more than 89 million
hospitalizations from 2017 to the third quarter of 2023, Medicare
Advantage beneficiaries experienced disproportionately greater
increases in extended hospital stays, especially among those
discharged to skilled nursing facilities.Meaning These findings suggest that the Medicare Advantage
plan design and practices may contribute to hospital discharge
delays, with implications for patient outcomes and hospital
capacity as enrollment continues to rise.

Ben Sommers and I have a new piece up at @jamahealthforum.com on how to think about generalizing lessons from unwinding to looming new Medicaid red tape requirements.

Serious overdose-related disability has not yet been a big focus of policy or research, but should be.

Furthermore, reducing unmonitored drug use, ensuring rapid overdose response, and expanding low-barrier access to medications for OUD may help prevent opioid overdose-related HIBIs.
pauljchristine.bsky.social
From 2016-2021, there were 22,360 hospitalizations with opioid overdose and hypoxic-ischemic brain injury (HIBI). Overdose-related HIBI hospitalizations were highly mortal (63% die), morbid, and costly, with nearly 50% being discharged to skilled nursing facilities, despite their young age
pauljchristine.bsky.social
Public health efforts are rightly focused on reducing opioid overdose deaths. We know relatively little about severe morbidity among overdose survivors. Our new paper in @journalgim.bsky.social provides the first national estimates of overdose-related hypoxic-ischemic brain injury

Our latest study @jamahealthforum.com shows the harms of work requirements for safety net programs. Women who were subject to harsher requirements and who received less cash assistance from TANF were less likely to breastfeed. jamanetwork.com/journals/jam... @donmoyn.bsky.social @hsph.harvard.edu 1/

We have new work out today at JAMA Health Forum exploring if removal of Medicaid prior auths for buprenorphine changed buprenorphine prescriptions

Hypothesis: PAs are a barrier to care and removal will result in more prescriptions

Results: There was minimal change after PA removal
