A new easy-read compliments and complaints form is being made available in GPs across Leeds.
Our team and people with learning disabilities helped make the form easy to use and understandable. This means that giving feedback to your GP will be easier for those who need it.
A page written in an easy-read format. GP practice compliments and complaints. You can use this booklet to tell your GP practice what they are doing well or to make a complaint.
