In fact, E/e' is a confounded measure; Epeak is a function of peak AV pressure gradient, not mean LA pressure, e' is a function of early LV compliance, which are related to relaxation and IVRT.
LV filling pressure: advices to keep it simple. Look at mitral flow (decT) and particularly lungs (LUS/CXR/high oxygen requirements). Points out that E/E’ is not validated in critical care, and does not track changes in pressure. #echofirst #EuroEcho #FOAMcc

EchoPro Tip #2: I call it the Arterial Positional Pulse Test: Palpate the radial pulse with the left arm at the patient’s side, then raise it overhead. In cases of severe AI, the pulse will disappear—a clinical clue when echo windows are impossible to get! #echofirst #cardiosky #medsky

If the LV of the patient with sepsis is «hyperdynamic» when on noradrenaline, then EF dropps and the LV looks poorly when vasopressin is added, but recovers to normal with dobutamine, you likely haven’t «uncovered hidden LV dysfunction» or septic cardiomyopathy. #emimcc #pocus #echofirst 1/

EchoPro Tip #1: A SHMOO-shaped septal LV screams coronary artery disease until proven otherwise, like in this obese smoker. I've seen so many SHMOO's end up having LAD or RCA blockages diagnosed after echo that I hope to one day see a "SHMOO-shaped" research article! #echofirst #medsky #cardiosky
