Today’s AHF pts are older, obese & multimorbid w/“inflammaging” rather than just pump failure. The latest SotA Review suggests early initiation of 4 GDMT pillars during AHF—and possibly added anti-inflammatory therapy—may improve longer-term outcomes & remission.

New science 🚨! In the BRING-UP-3 HF study, among ambulatory HF pts in Italy, 65% of HFrEF pt received all 4 GDMT pillars. BBs & RASi/ARNI >90%, MRAs & SGLT2i >80% (though target doses still low). SGLT2i already common in HFmrEF (72%) & HFpEF (50%)!

Despite strong evidence, GDMT remains underused. In this Indian expert consensus doc, using a modified Delphi process, authors propose simplified, patient-profile–based guidance to enable early, safe initiation of the 4 GDMT pillars & improve outcomes in HFrEF.

💭 Should invasive pressure-volume loops be incorporated into diagnostic testing for HFpEF?

‼️Half of pts w/HFpEF can be missed w/RHC even w/exercise! And diagnostic scores for HFpEF have limited sensitivity.

But invasive LV pressure-volume loops can help!

Benefits of ICDs & CRT-Ds⚡️ in HM3 LVAD pts is uncertain.

This analysis of the MOMENTUM-3 trial showed that the presence of these devices was not associated with ⬆️survival, quality of life, or functional capacity in HM3 LVAD pts.

In this VA study, the rates of early 🫀follow-up after HFH were low overall & were only modestly associated w/mortality risk at discharge.

Increasing the visit rate after HFH should be evaluated as a mechanism to improve outcomes after HFH!
