High serum aluminum with toxicity from sucralfate

The -al- in sucrALfate is aluminum

I had a patient on sucralfate (severe GI reasons) whose PO4 would be pristine. Aluminum is a fantastic PO4 binder (but toxic to may tissues)

"Sure enough, the lab reported 'toxic' levels of Aluminium in my blood. While the normal range is < 15 mcg/L, my level was 73 mcg/L. Above 50 mcg/L was considered toxic."

After a few years with a chronic illness like kidney faillure, it is not death that one worries about but suffering. Most people in this situation would take a painless, instant death in a heartbeat.

When an adverse event happens in a healthcare setting, the first impluse of most HCWs is to worry about how their superiors or the patient's family would react and not what the impact of the event is on the patient.A truly patient-centric org should strive to change this mindset.

How much longer before "Your kidneys have failed, get a fistula" changes to "Your kidneys have failed and these are your 4 options"? Patients need to be part of decision making that is going to dictate the rest of their lives. Not blindly accept the default. #SharedDecisionMaking @nephroplus
