🧵2 words likely to trigger most ID/ASP clinicians = ‘asymptomatic bacteriuria’ (ASB). 😱 Except for pregnant persons & those undergoing certain urologic procedures, there's no 🚫 clear evidence of benefit of treatment & some signal for harm in other populations. #IDSky #RxSky #MedEd #PharmEd 1/n

Happy Holidays from me and Echo 🩵🎄🐈

Pets of #IDSky #PharmSky #SIDP #MedSky
Echo the orange cat standing next to a Charlie Brown-esque potted fir tree. The tree is decorated with multi-colored lights, royal blue and pearl white ball ornaments, and a snow flake ornament towards the top.

Echo the orange cat asleep on a light blue fluffy blanket, the picture of ultimate rest and relaxation. He is laying on his side facing the camera and pink toe beans on one of his front paws are peaking out from folds of the blanket.

Taken by surprise by this single bloom on one of my Christmas cacti today. I can’t find another bud on the entire plant 🪴 💚
Pale, pale yellowish bloom on a Christmas cactus (maybe a Thanksgiving cactus 🤷🏼‍♀️). Hot pink stamen (pretty sure that’s the stamen?). No other buds to be found on the entire plant. A pleasant surprise. Pale, pale yellowish bloom on a Christmas cactus (maybe a Thanksgiving cactus 🤷🏼‍♀️). Hot pink stamen (pretty sure that’s the stamen?). Echo wishes you some rest on this #Caturday 😴🐈

Echo the orange cat, stretched out on the bed, head snuggled into my forearm, eyes closed, content, front legs reaching towards me, white whiskers catching the light coming in through the gap in the curtains.

High rates of Mycoplasma pneumoniae ("walking pneumonia") in Ontario, 2024, especially in those age 5-17.

16.3% of a subset of 135 specimens had mutations that suggest resistance to macrolide antibiotics.

Some great references below on UTI/ASB! #IDSky #UTISky

Also, please make the “UA positive for UTI” diagnosis go away 😭

There are so many awesome studies in this area - what are some of your favorites?
meganismofaction.bsky.social
Hopefully these data will help reframe the risk-benefit ratio of ASB treatment. Evidence suggests risks of not treating are less than feared & perhaps treating may actually increase risk of subsequent symptomatic UTI by killing off colonizing ASB strains which may have a protective role. 10/10 🔚
meganismofaction.bsky.social
Might this ability of ASB strains to outcompete uropathogens explain the signal in the literature for *increased* risk of symptomatic UTI following ASB treatment? 🤔 6/n

pubmed.ncbi.nlm.nih.gov/26270684/
Kaplan-Meier curves at 12 and 40 months of follow-up, showing that patients receiving asymptomatic bacteriuria treatment were more likely to develop subsequent symptomatic UTI.
meganismofaction.bsky.social
There are well-characterized ASB strains, like E. coli 83972, that lack virulence factors such as adhesins and are distinct from uropathogenic E. coli. 4/n
pubmed.ncbi.nlm.nih.gov/17502385/
Figure 2 from linked article that contrasts an asymptomatic bacteriuria (ASB) strain of E. coli, E. coli 83972, and uropathogenic or UPEC E. coli. The ASB strain lacks adhesins and therefore cannot ascend to the upper urinary tract. It maintains urine colonization due to its fast growth and potentially also biofilm formation. Host immune response is not triggered in response to these ASB strains.
