I remember when I was unaware of this conundrum of applying the gold standard of RCT to studies where outcomes are potentially years in the future, as well as being relatively small so require large populations. I was actually wondering if simply overlooked (while also trying to write in lay terms)

I wanted to highlight when a placebo- or comparator-controlled randomized trial might not apply, as you are correct that they are usually considered a “gold standard”. (Multiple posts because trying to be clear, especially if others with less background read the posts. And because of word limits.)

Recruitment limited to specific people might also be biased according to the type of people who would volunteer for such a study and therefore not necessarily reflect the demographics of the general population

And then there is the issue of ethics—there is potential harm to a placebo group who do not receive an effective vaccine. Perhaps there are people who would enroll with full-informed consent (for personal or other reasons), but a double-blind trial would be difficult to recruit to the level required

And my admittedly weak understanding of vaccine trials is that there already are initial studies designed to assess safety signals in the short term, albeit in relatively small groups compared to millions who might receive vaccines

Vaccines prevent disease, so a randomized trial with evidence of disease, let alone survival, would require many years with many, many patients to potentially see a statistically-significant signal

This issue arises often in cancer studies, primarily in studying the effectiveness of drug treatments in early-stage solid tumors where surgery (often followed by radiation) is extremely effective. Additional use of drug is essentially to “prevent” recurrence, so outcomes can require years to study

Not sure the context in which the original comment was made, but long-term studies, especially for prevention, can take years. Another issue with lengthy studies is that science continues to evolve, so a comparator might be “standard” when study started but not remain so over time
